If you have a diagnosis, that is the more useful starting point, because the condition is what a clinician documents and what an administrator reads. The product follows from it. Someone managing migraines and someone managing chronic fatigue can end up buying completely different things and both have a legitimate claim.


Each guide here takes a condition and works outward: what tends to get documented, which purchases clinicians commonly connect to it, what the strongest version of the letter says, and which purchases people assume will qualify but usually do not.


Pregnancy and postpartum sit here too. They are not conditions, but they run on the same logic, and they are two of the periods where people spend the most and claim the least.
